Get Screened

You can take a self-test in a variety of places, ranging from healthcare settings such as local clinics to non-healthcare settings for example youth venues, drop-in centres and certain pharmacies.

Self- testing postal kits are also available and can be requested via the website or by telephoning the Chlamydia Screening Office on 01922 270554 or 01922 270556.

Follow the simple steps below to find out if you have Chlamydia

Males

  1. Read the National Leaflet for Chlamydia Screening provided with the test request form.
  2. Complete the test request form, making sure you enter your postcode and identify how you wish to be notified of your results (if you do not complete these details, it may mean your sample is not tested).
  3. The information you give will be treated in the strictest confidence. You have the right to refuse to answer any of the questions on the form, but we encourage you to complete it to the best of your ability.
  4. Tear off the front sheet and keep this safe as it has important information about what your test results mean.
  5. Attach one of the numbered ‘sticky labels’ found attached to the form to your urine specimen container (this is your unique screening number).
  6. Go to the toilet and pee in the pot.
  7. Secure the lid tightly, place the specimen in the plastic bag attached to the test request form and seal the bag.
  8. Return the specimen to the site where you picked it up from (if you have a postal return kit, place in the protective packaging enclosed and return it via the pre-paid envelope).
  9. Please return or post your specimen on the day you take it.

It’s as simple as that!
